In Northern Ireland, Macmillan is working with the Health and Social Care Board, Public Health Agency and Northern Ireland Cancer Network to provide individually tailored patient after care and support. We have invested one million pounds in the three year project.
The results have been encouraging. In fact, the Transforming Cancer Follow Up programme has been adopted by Northern Ireland’s five Health Trusts as the new model for follow up breast cancer care.
As a result, more than 40% of all newly-diagnosed breast cancer patients are now moved onto self directed after care, which has reduced the number of routine review appointments.
Patients are taught what to look out for, given their own individual care plan, invited to a health and wellbeing event, and told how to ensure they are seen quickly, should any problem occur.
