Transforming cancer and end of life care - image description

The current system for cancer and end of life patients is shown on the left hand-side of the diagram – with poor coordination between the providers of healthcare which means the patient can experience unnecessary hospital admissions and poor quality of care. On the right is what the Transforming Cancer and End of Life Care programme is hoping to achieve; a more streamlined, patient-focused system where there is better communication between health service providers. As a result the patient will have a better experience, we will have better survival rates and it will be a system which can cope with the increasing numbers of people being diagnosed with cancer.
