Important forms

From medical details to extending your stay in Peru, these are the important forms you will need to complete and return to take part in the Peru Hiking Challenge.

• Medical form: Please send to Discover Adventure (DA) by 4 November 2012
• Discover Adventure insurance form: If purchasing the DA insurance please send  this form to DA by 4 March 2013
• Own Insurance form: If using an existing policy please send this form to DA by 4 March 2013
• Flight extension form: If you plan to extend your stay in Peru after the challenge please send this form to DA by 4 February 2012
